How to Obtain
The Stone-Sheathed Sword can be obtained in three different locations, but can only be obtained once per New Game-cycle. If you pick up the item at one of the three altars, the other two can be used to convert the weapon into the Sword of Light or the Sword of Darkness respectively.
Gravesite Plains - Found in front of an altar in the deeper parts of the Fog Rift Catacombs. To access, descend into the catacombs, until you reach a room with three trapped ceilings. Make your way to the end of the room and jump unto the contraption and ride up. From this point you can walk across to reach the altar.

Ruins of Unte - Found in front of an altar in the blocked courtyard of the Ruins of Unte. You need to wake up the Furnace Golem blocking the way to the church access the item. To do this, throw a Hefty Fire Pot or Hefty Furnace pot at it's face from a higher point.

Ancient Ruins of Rauh - Found in front of an altar. To access this area travel Northwest from the Rauh Ancient Ruins, West site of grace and enter the temple. Travel through the scarlet rot area while keeping northwest. After exiting the ruins you can see the tower ahead which can be easily accessed by an invisible, straight bridge. The altar is on top of said tower.
